29 CFR  1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:  a) Jobsite located at 1307 Germantown Ave, Philadelphia, PA - On or about July 27, 2022, the employer did not ensure employees engaged in residential construction were protected by a guardrail, safety net, or personal fall arrest system. Employees were placing building materials on the roof of a four story residential building adjacent to an unprotected edge, approximately 40 feet from the ground level.
