29 CFR 1926.451(c)(2)(v):  The employer used forklifts to support scaffold platforms without ensuring that the entire platform was attached to the fork and that the forklift was not moved horizontally while the platform was occupied:  Employees engaged in construction activities at the Pizza Hut restaurant located at 707 N Diers Ave, Grand Island, Nebraska, were exposed to a fall hazard in that a supported scaffold platform that was being used with a powered industrial truck was not secured to the forks.

29 CFR 1926.451(g)(1):  Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  Employees engaged in construction activities at the Pizza Hut restaurant located at 707 N Diers Ave, Grand Island, Nebraska, were exposed to a fall hazard in that employees did not have fall protection while working on a mobile scaffold that was approximately 16 feet above the ground.

29 CFR 1926.501(b)(10):  Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, warning line system and personal fall arrest system, or warning line system and safety monitoring system; or, on roofs 50 feet (15.25 m) or less in width, the use of a safety monitoring system alone:   An employees engaged in roofing activities at the roofing site located at 1071 707 N Diers Ave, Grand Island, Nebraska, were exposed to a fall hazard in that the employee was working on a low-slope roof that was approximately 16 feet from the ground without a safety monitor or fall protection.

29 CFR 1926.454(a):  The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards as required by paragraph (a)(1) of this section:                                                                                                   Employees engaged in construction activities at the Pizza Hut restaurant located at 707 N Diers Ave, Grand Island, Nebraska, were exposed to a fall hazard in that employees did not have fall protection while working on a mobile scaffold that was approximately 16 feet above the ground and were not trained to the nature of fall hazards as a result of the condition of the scaffold that had a lack of fall protection.
