29 CFR 1926.25(a):During the course of construction, alteration, or repairs, form and scrap lumber with protruding nails, and all other debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures: At the Gateway Woods apartment project employees were exposed to trip hazards and puncture hazards in the following instances: a) Building 5, where piles of boards that had been used as bracing, some with nails sticking up, were strewn around the perimeter of the building; b) Building 3, where piles of boards and other discarded building materials covered the ground outside to the building; and c) Building 2, where the ground was littered with construction debris, some with protruding nails.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system: At the Gateway Woods apartment project, on building #5 where employees of the roofing contractor were installing shingles without utilizing any form of fall protection. This practice exposed the employees to the hazard of falling from the roof to the ground below.
