29 CFR 1926.404(f)(6):The path to ground from circuits, equipment, or enclosures was not permanent and continuous: At the jobsite, building 4, where masons used extension cords that were missing the equipment grounding conductor to provide power to the grounding-required DeWalt brick saws in the following instances: a) Extension cord number one with a clear male end; b) Extension cord number two with a blue male end; and c) Extension cord number three with a black male end. This condition exposed the employees to electrical shock hazards.
