29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: a) On or about 08/02/2022, at 6874 Avalon Rd. Winter Garden, FL 34787, employees were exposed to crushing and suffocation hazards when they were observed installing piping in the bottom of a trench that measured 13' deep 30' wide and 55' long on one side of the L and 13' deep 30' wide and 32' long on the other side, with the only means of egress being a broken ladder that was not long enough to reach the top of the excavation. No abatement certification or documentation required for this item.

Winter Garden, FL 34787, employees were exposed to struck by hazards hazards from falling equipment and debris when they were observed installing piping in the bottom of a trench that measured 13' deep 30' wide and 55' long on one side of the L and 13' deep 30' wide and 32' long on the other side. The spoils piles and the Deere 350G excavator were located within 2 feet of the edge without any protection to prevent them from rolling into the excavation. No abatement certification or documentation required for this item.
