29 CFR 1910.303(b)(1)(iv):Electric equipment was not free from recognized hazards that were likely to cause serious physical harm to employees, such as damaged electrical insulation: At a commercial worksite located at 223 W Splitlog Rd, Goodman MO -- employees were using a fan to cool off that had been spliced with wire nuts. The insulation had been cut away causing the wires to be exposed. This exposed employees to electrical burns/shocks. This was documented on or about 08/02/2022.
