29 CFR 1926.21(b)(2):The employer did not instruct each employee in the recognition and avoidance of unsafe condition(s) and the regulation(s) applicable to his work environment to control or eliminate any hazard(s) or other exposure to illness or injury: a) On or about August 2, 2022; the employer exposed employees to struck-by and crushing hazards in that the employer failed to instruct its employees engaged in sewer pipe installation in the recognition and avoidance of unsafe conditions applicable to their work environment to control or eliminate any hazards or exposure to illness or injury. Employee(s) were allowed to work on a site with heavy equipment such as but not limited to excavators without first being trained in the requirements of staying clear of the equipment while in the equipment was in operation.
