29 CFR 1926.452(i)(4):Outrigger beams were not secured in place against movement, and were not braced at the fulcrum point against tipping. a. At the site SIMCOM, stairway one west: On or about July 29, 2022, and at times prior, the employer exposed employees to fall hazards from approximate 8 to 10 feet in height, in that outriggers were not used to provide stability to the baker scaffold while employees insulated pipes.
