29 CFR 1926.451(c)(1): Supported scaffolds with a height to base width (including outrigger supports, if used) ratio of more than four to one (4:1) were not restrained from tipping by guying, tying, bracing, or equivalent means: a) Jobsite, 1144 Kirkbride Street, Pittsburgh, PA: On or about August 3, 2022, during brick laying work, the employer did not ensure that the scaffolding, which was 4 frames long on one side of the building, 6 frames long on the other side of the building, at least 26 feet tall with a least base dimension of 5 feet 2 inches, was properly protected from tip over hazards. The employer installed 2 bar braces, one joining the scaffold frames together at the top corner and the other near a lower doorway, but took no other actions, such as installing guys or tie-ins, to stabilize and prevent the scaffolding from falling over.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms: a) Jobsite, 1144 Kirkbride Street, Pittsburgh, PA: On or about August 3, 2022, during bricklaying work, the employer did not ensure that appropriate fall protection was in use along the sides and end of the scaffolding at the top of the scaffold, exposing an employee to a fall hazard of at least approximately 26 feet. b) Jobsite, 1144 Kirkbride Street, Pittsburgh, PA: On or about August 3, 2022, during bricklaying work, the employer did not ensure that appropriate fall protection was in use along the sides of two frames/bucks of the scaffolding near the mud tub area, exposing an employee to a fall hazard of at least approximately 13 feet.

An employee was observed climbing down various scaffold components in order to get down.
