29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) At 1701 Belfast River Road, Richmond Hill, GA, on or about August 3, 2022, and at times prior to, the employer exposed employees to fall hazards greater than 9 feet, in that the extension ladder the crew used to gain access to the roof did not have the side rails of the ladder extended at least three feet above the landing to provide safe access to the roof.
