29 CFR 1910.151(c):Where the eyes or body of any person may be exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body shall be provided within the work area for immediate emergency use. In July 2022, at an establishment located at 3015 North Main Street, in Oshkosh, Wisconsin; adequate eyewash station(s) were not provided when employee(s) are verifying the water levels in the batteries of the electric powered forklifts exposing employees to potential eye injuries.

29 CFR 1910.178(l)(2)(ii): Training shall consist of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace. The employer did not conduct the practical training portion as part of the overall powered industrial vehicle training program for all employees who were trained and certified to operate the powered industrial vehicles.
