This was most recently documented on August 4, 2022 at a residential construction jobsite located at 1121 Dorsey Street, Beatrice, Nebraska when employees were observed working on a residential structure with a roof pitch less than 4:12. The employees where installing shingles while working at a height of approximately 9 feet measured from the ground to eave without fall protection.

This was most recently documented on August 4, 2022 at a residential construction jobsite located at 1121 Dorsey Street, Beatrice, Nebraska when employees were observed working on the roof of a residential structure. The ladder used by the employees was not extended over the upper landing surface.
