Production, PL 101 - On or about July 27, 2022, an employee sustained a right middle fingertip amputation while cleaning the Line 1 Hopper. The employer did not ensure that an energy control procedure was developed and utilized for the control of hazardous energy when employees are engaged in the service and/or maintenance task of cleaning the hoppers.

29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation: a.) Production, PL-101 - On or about July 27, 2022, the employer failed to provide training to each authorized employee performing service and/or maintenance to machinery or equipment to ensure the employee acquired the knowledge and skills required for the safe application, usage and removal of energy control devices. A production sanitation employee sustained right middle fingertip amputation while cleaning the Line 1 hopper.
