29 CFR 1926.453(b)(2)(v): A lanyard was not attached to the boom or basket when working from an aerial lift: On or about 08/04/2022, the employer did not ensure that employees working in an aerial lift connected into the boom or basket by use of a fall protection lanyard. Employees were exposed to the hazard of falling when: A. On the left front of the building, two (2) employees were working in the basket of a JLG 460SJ aerial lift and where not connected by use of a lanyard. B. On the middle front of the building, two (2) employees were working in the basket of a JLG 460SJ aerial lift and were not connected by use of a lanyard. C. On the right front of the building, two (2) employees were working in the basket of a JLG 600S aerial lift and were not connected by use of a lanyard.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On or about 08/04/2022, the employer did not ensure fall protection was implemented for employees performing construction work on a roof. Employees were exposed to fall hazards associated with an approximately ten (10) foot fall.
