29 CFR 1910.147(c)(4)(i):Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a)Viaflex , Inc. at 821 West Algonquin in Sioux Fall, SD 57103: On or about and at times prior to July 29, 2022, the employer did not develop, document and utilize procedures that ensure employees were protected from hazardous mechanical energy associated with ingoing nip-points when servicing the Line 14 Turret Winder (Macro ME-12-2 Turret Winder Job No. RVI - 13636). An employee suffered abrasion and compression injuries to their upper body after they became caught between the core and lay-on roller.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by ingoing nip points: (a)Viaflex at 821 West Algonquin in Sioux Fall, SD 57103: On or about and at times prior to July 29, 2022, the employer did not ensure employees were protected from an ingoing nip-point created by movement of the shaft and lay-on roller of the surface winder (Brampton Engineering Model SW94A-LU Serial Number: 21651-21B Winder B) by Line 9 by appropriate guarding. This condition exposed employees to compression and abrasion hazards.
