29 CFR 1910.180(h)(3)(i)(a): The employer shall assure that the crane is level and where necessary blocked properly. On or about July 29, 2022, at 310 Tyler Court in Menasha, Wisconsin, an employee who was operating a truck crane unloading pallets of roofing materials was exposed to crushing and struck-by hazards when one outrigger was set-up on top of a debris tarp prior to inspection of the surface. The crane outrigger was not blocked properly and sank into the soft soil resulting in it tipping into the structure.

29 CFR 1910.180(h)(3)(xiv): Before traveling a crane with load, a designated person shall be responsible for determining and controlling safety. Decisions such as position of load, boom location, ground support, travel route, and speed of movement shall be in accord with his determinations. On or about July 29, 2022, at 310 Tyler Court in Menasha, Wisconsin, an employee who was operating a truck crane unloading pallets of roofing materials was exposed to crushing and struck-by hazards when one outrigger was set-up with insufficient ground support and it sank into a soft soil surface. The employee did not determine the condition of ground support prior to lifting the load.
