29 CFR 1926.501(b)(1): Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: Location: 88 Grove St, Monsey, NY 10952 a) On or about August 3rd, 2022, the employer did not ensure that employees use appropriate fall protection while engaged in commercial construction activities 6ft or more above lower levels. b) Employees were exposed to fall hazards, approximately 30ft, from the ground surface below, when walking on and working from a commercial structures' roof without use of fall protection.

29 CFR 1926.501(c): When employees were exposed to falling objects the employer did not require employees to wear hard hats and did not implement measures to prevent objects from falling from higher levels as set forth in (c)(1)-(3) of this section: Location: 88 Grove St, Monsey, NY 10952 a) On or about August 3rd, 2022, the employer did not ensure employees were wearing hardhats while engaged in commercial construction from falling objects approximately 30ft or more above lower levels. b) An employee was working on a ladder and was struck-by a falling object on the head while not wearing a hardhat on the construction site.
