29 CFR 1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): On or about August 3, 2022 at the Robbie Road Re-Roofing Project, employees were removing the old shingles from several roof decks and installing new plywood and a new roofing system and no fall protection was in use. D&M Best Homes and Construction LLC was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1559927, citation number 01, item number 001 and was affirmed as a final order on 11-24-2021, with respect to a workplace located at 94 Preston Street in Windsor, CT.

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access; and, when such an extension was not possible because of the ladder's length, the ladder was not secured at its top to a rigid support that would not deflect, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladder. On or about August 3, 2022 at the Robbie Road Re-Roofing Project, employees were using extension ladders to access the roof decks and the ladders did not extend at least 3 feet past the landing. D&M Best Homes and Construction LLC was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1559927, citation number 01, item number 002 and was affirmed as a final order on 11-24-2021, with respect to a workplace located at 94 Preston Street in Windsor, CT.
