29 CFR 1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:  The employer is failing to protect employees from fall hazards while working from height during residential roofing operations. This was most recently observed on August 4, 2022 at 2312 South 35th Ave, Omaha, Nebraska when three workers were installing roofing materials to a residential structure that had an eave height of 10 feet 9 inches above the ground, and the workers were not using fall protection.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet above the upper landing surface being accessed:  The employer is failing to protect employees from fall hazards while using ladders to access elevated surfaces.  This was most recently observed on August 4, 2022, at 2312 South 35th Ave, Omaha, Nebraska where employees were using a ladder to access the roof of a residential building and the ladder's side rails did not extend three feet past the eave of the roof.
