The exit door was located in the Northeast area of the facility. ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

PSP Stores, LLC dba Pet Supply Plus, Breakroom/storage area, on or about July 29th, 2022 and times prior there to: Items including, but not limited to, a Phoenix-Titan brand floor scrubber was stored directly in front of electrical panel "H". The electrical panel box was a Siemens brand 480Y/277 3 phase electrical panel box. b.) PSP Stores, LLC dba Pet Supply Plus, Breakroom/storage area, on or about July 29th, 2022 and times prior there to: Items including, but not limited to, a carboard box with fluorescent light bulbs, a plastic storage bin, and ice melt were stored directly in front of two electrical panel boxes "A" and "B". The electrical panel boxes were Siemens brand 120/208 Volt 3 phase electrical panel boxes. ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.
