Employees used a Rolair air compressor where the power cord was missing the ground prong, thereby exposing employees to 119-volt electrical contact hazards. b) B H & D CONSTRUCTION LLC located at Glassboro Street, Lot 1980, Newark, Ohio 43055: On and preceding August 2, 2022, the employer did not ensure employees used extension cords that had a continuous path to ground. Employees used a DeWalt air compressor where the power cord was missing the ground prong, thereby exposing employees to 119-volt electrical contact hazards.

Framers were working on the second story of a two-story residential home at heights up to 28 feet. The employees were not utilizing any form of fall protection which exposed them to falls hazards of 28 feet.
