29 CFR 1910.22(a)(2): The employer did not ensure that the  floor of each workroom is maintained in a clean and, to the extent feasible, in a dry condition. The employer did not ensure that when wet processes are used, drainage is maintained and, to the extent feasible, dry standing places, such as false floors, platforms, and mats are provided.  a) For the floor of the cooler/vault area, on or about 08/05/2022, the employer exposed employees to slip/fall hazards, in that, the floor was not clean and dry; rather it was coated in areas with milk and milk/fat deposits where employees walked/worked.

29 CFR 1910.22(d)(1): The employer did not ensure that walking-working surfaces are inspected, regularly and as necessary, and maintained in a safe condition  a) For the floor of the cooler/vault area, on or about 08/05/2022, the employer exposed employees to trip/fall hazards, in that, there was a 2-3 inch wide floor hole next to a moving floor conveyor that employees could step in.  b) For the floor of the cooler/vault area, on or about 08/05/2022, the employer exposed employees to trip/fall hazards, in that, there was an open drain hole next to a moving floor conveyor that employees could step in.

29 CFR  1910.147(d)(3):All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s):  a) For the cooler room, on or about 08/05/2022 and at times prior to, the employer exposed employees to potential amputation and electrical hazards, in that, push buttons and manual motor starter circuits were used to de-energize chain drive motors for maintenance/servicing work rather than circuit breakers which were located in the MCC room. Maintenance/servicing work consisted of activities such as, but not limited to, changing motors, belts, chains and sprockets. The motor starter circuits and push buttons were not designed to be used as energy isolating devices for lockout tagout.
