29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s): a) Crofton Construction Services: On or about 08/02/2022, the mechanical/electrical power to the Air Conditioning unit of the Manitowoc 4100 crawler crane, the energy isolating devices were not secured as a result an employee's right index finger slipped between a pulley and received a severe laceration. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
