Splitrail Ave. - On or about August 8, 2022, employees installing shingles accessed a 7:12 pitch roof with the use of a multi-use folding ladder which was not extended at least three (3) feet above the roof's edge, exposing employees to fall hazards greater than nine (9) feet to the ground level below.
