On or about August 8, 2022, the employer failed to remove  forklifts  from service after multiple deficiencies had been identified. The Yellow- Hyster Fortis 120 lift truck model S120FTPRS Serial No. G004V06507K  did not have a functioning seatbelt and badly worn tread on the tires (rear tires) .  The Mitsubishi - F140 Serial No. AF19B-00468  Forklift did not have a functioning horn and a faulty left  top headlight. These conditions exposed employees working in the area to struck-by hazards.  No abatement certification or documentation is required for this item.
