29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system:  Employees engaged in framing activities at the jobsite located at 3306 Big Elk Parkway, Elkhorn, Nebraska were exposed to fall hazards in that a framer working on the top of a residential site was not utilizing fall protection.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards, which would enable each employee to recognize the hazards of falling and train each employee in the procedures to be followed in order to minimize these hazards:  Employees engaged in framing activities at the jobsite located at 3306 Big Elk Parkway, Elkhorn, Nebraska were exposed to fall hazards in that the employees were not trained in the recognition of fall hazards and the methods to mitigate these hazards.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  Employees engaged in framing activities at the jobsite located at 3306 Big Elk Parkway, Elkhorn, Nebraska, were exposed to fall hazards in that side rails of a portable ladder that was used for access to an upper landing surface did not extend at least 3 feet above the upper landing surface being accessed, and the ladder was not secured at its top to a rigid support.
