At 11801 1st Street East, Treasure Island, FL., on or about 08/05/2022, employees in the process of roofing activities, on a 5 in 12 slope (vertical to horizontal) roof, were exposed to a 36 foot fall hazard, in that, fall protection equipment was not installed or utilized.
