29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws: On or about August 9, 2022: a. In the fabrication shop area, located on the north wall, employees were exposed to electrical hazards while operating a Dart brand pedestal control (SN D19-02238) with a flexible cord that did not have a strain relief device installed. b. In the fabrication shop area, located on the north wall, employees were exposed to electrical hazards while operating a stainless-steel positioner with a flexible cord that did not have a strain relief device installed.

29 CFR 1910.334(a)(2)(ii): There was a defect or evidence of damage that could have exposed an employee to injury and the defective or damaged item was not removed from service and an employee was using it before repairs and tests necessary to render the equipment safe were made: On or about August 9, 2022, in the Tower Field service building, center area fabricated metal area, employees were exposed to electrical hazards while operating a Dewalt brand 14 inch Chop Saw (SN 813525), with the power cord that was damaged and taped.
