29 CFR 1926.451(f)(6): Scaffolds were erected, used, dismantled, altered, or moved such that they, or any conductive material handled on them, could have come closer than the minimum distances established for specific voltages as set forth in the table in subparagraph (f)(6). (a) On the front north side of the house: On or about 8/10/2022, employees were not protected from energized electrical circuits of approximately 220 volts while working from a metal pump-jack scaffold system that was set up within inches of a residential power line.

29 CFR 1926.416(a)(3): When and where energized circuits existed, the employer failed to post and maintain proper warning signs, advise employees of the location, hazard involved and the proper protective measures to be taken. (a) On the front north side of the house: On or about 8/10/2022, the employer failed to post and maintain warning signs and advise employees of the location and hazards of live electrical lines during the set up and use of a pump jack scaffold with an aluminum plank and an aluminum extension ladder that was within inches of an energized electrical line carrying approximately 220 volts.
