On or about August 8, 2022, the employer did not ensure that the compressed gas cylinders located in the machine room were handled and stored in accordance with the Compressed Gas Association (CGA) Pamphlet P-1-1965 due to no poster to display the names of the gases stored (3.3.2), Oxygen, acetylene and nitrogen cylinders stored together without separation (3.3.3), fully charged and empty compressed gas cylinders stored together (3.3.4), and cylinders were stored near open containers of flammable oils and solvents (3.3.6).
