29 CFR 1926.25(a): Throughout this job site, during the course of construction operations, framing materials with nails, pipe and trash were lying in and adjacent to walkways and work areas, exposing employees to trip and fall hazards. Location - 17 Cherry Street, Medford, MA. On or about August 10, 2022, three employees were exposed to protruding nails and trip hazards from demolition and framing debris that were not disposed of in a timely manner.

On or about August 10, 2022, employees were not adequately protected against falls of 6 feet or greater above a lower level in the following instances: Instance a: Three employees were exposed to fall hazard greater than sixteen feet above lower level while working/walking on the third floor deck. Instance b: Three employees were exposed to fall hazard from two open floor holes on the second floor level. Instance c: Three employees were exposed to fall hazards greater than sixteen feet above lower level while working/walking at or near unguarded window and door openings.
