29 CFR 1926.453(b)(2)(v): A body belt with a lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift. On August 10, 2022, on the south side of the site, employees used an aerial lift to access the roof of a building and were not in use of fall protection. This exposed the employees to a fall hazard greater than six (6) feet to the ground.

29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems. On or about August 10, 2022, on the south side of the site, employees were performing roofing work on a metal roof without the use of fall protection. The employees were exposed to a fall greater than 15 feet above the lower level.
