On or about August 10, 2022, on the west side of the site, an employee was not in use of fall protection when working from the roof of a building. This exposed the employee to a fall greater than six (6) feet to the ground below.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access.  On or about August 10, 2022, on the west side of the site, an employee used a ladder to access the roof and the ladder side rails did not reach the roof edge and did not extend at least three feet above the landing surface.  This exposed the employee to a fall hazard.
