1910.132(f)(3)(iii) The employee's knowledge or use of assigned personal protective equipment indicates that the employee does not have retained the requisite understanding or skill. a) RK Mechanical, 3800 Xanthia St, Denver, CO 80207: On or about 8/8/2022, the employer did not ensure employees were protected from point of operation hazards while using a metal former to form the bead in the material. The RAS easy Former, 12.35-2 in the "Rounds" area was not guarded at the point of operation while the employee was forming a piece of material that was not uniform in shape, inherently changing the feed distance and leading to the employee's finger to be caught in the point of operation. This exposed employees to amputation and point of operation hazards.
