29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use: a) In Building B, on or about August 10, 2022 - Employees pour Thinner Norilit U 90 from a one liter container into a Dixie cup to mix ink for the pad printer. The personal wash unit provided by the employer in the work area was not suitable because it can not be used without requiring the use of the operator's hands (once activated), it did not have a sufficient volume of flushing fluid to supply flushing for a minimum 15-minute period, and the flushing fluid expired November 2016.

29 CFR 1910.1200(g)(1): Employers did not have a safety data sheet in the workplace for each hazardous chemical which they use: a) In Building A, on or about August 10, 2022 - The employer did not have a safety data sheet in the workplace for the Rustlick WS-5050 Water-Soluble Oil that employees use as a coolant in the CNC lathe. The material safety data sheet the employer maintained for Rustlick WS-5050 was revised on August 3, 1995 and lists Triethanolamine (CAS #102-71-6) as a reportable component. The Rustlick WS-5050 Water-Soluble Oil that employees use contains Petroleum distillates hydrotreated heavy naphthenic (CAS #64742-52-5), Water (CAS #7732-18-5), Chlorinated paraffins (CAS #1372804-76-6), Tall Oil (CAS # 8002-26-4), Ethoxylated Alcohols (CAS #68920-66-1), and 1,3,5-Triazine-1,3,5(2H, 4H, 6H)-triethanol (CAS #4719-04-4). b) In Building B, on or about August 10, 2022 - The employer did not have a safety data sheet in the workplace for the Thinner Norilit U 90 that employees use as an ingredient in the ink that they mix for the pad printer. The material safety data sheet the employer maintained for Thinner Norilit U 90 was revised on July 8, 1996 and lists Cyclohexanone (CAS #108-94-1) as a component. The Thinner Norilit U 90 that employees use contains 2-butoxyethyl acetate (CAS #112-07-2), Cylohexanone (CAS #108-94-1), and propylene carbonate (CAS #108-32-7). c) In Building B, on or about August 10, 2022 - The employer did not have a safety data sheet in the workplace for the Dichloromethane that employees use to join two plastics together. The material safety data sheet the employer maintained for Dichloromethane lists the manufacturer as PPG Industries, Inc. The Dichloromethane that employees use is manufactured by EKI.

Rustlick WS-5050 Water-Soluble Oil is a Category 1 skin sensitizer. b) In Building B, on or about August 10, 2022 - Employee training did not include the physical and health hazards of Thinner Norilit U 90 that employees use as an ingredient in the ink that they mix for the pad printer. Thinner Norilit U 90 is a corrosive, combustible liquid, harmful if swallowed, causes skin irritation, and causes serious eye damage. c) In Building B, on or about August 10, 2022 - Employee training did not include the health hazards of Dichloromethane that employees use to join two plastics together. Dichloromethane, is harmful if swallowed, causes skin irritation, causes serious ere irritation, suspected of causing cancer, may cause respiratory irritation, and may cause drowsiness or dizziness.
