29 CFR 1926.302(b)(1):Pneumatic power tool(s) were not secured to the hose or whip by some positive means to prevent the tool from becoming accidentally disconnected: On or about 08/08/2022 employees at the South Hartford Conveyance and Storage Tunnel were exposed to struck-by hazards. An employer provided PHQ 250 Pneumatic Rock Drill, was not provided with a means to prevent an accidental disconnect of the pressured air hose.
