29 CFR  1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use:   (a) On or before 7/28/22 the employer did not ensure that employees used equipment designed to safety convey personnel to elevated work surfaces. Employees rode a stack of plywood supported by a rough terrain forklift to access the porch roof of a new townhome under construction. This condition exposed employees to fall hazards greater than 6 feet.   Abatement Note: Abatement Certification is required for this item. See enclosed "Certification of Corrective Actions Worksheet."

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a) On or before 7/28/22 the employer did not ensure that employees were protected from fall hazards while sheathing the porch roof of a townhome building under construction. This condition exposed employees to fall hazards greater than 9 feet.  Abatement Note: Abatement Certification is required for this item. See enclosed "Certification of Corrective Actions Worksheet."
