29 CFR  1910.178(p)(1):  Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition.  On or about 8/10/22, Blastco, Inc., had not ensured any powered industrial truck which was found to be in need of repair, defective, or in any way unsafe, was removed from service. The following forklifts were in use and not removed from service:  a)  Kalmar P100CX, SN-103211A. The forklift was defective at the time of the inspection; it was missing a  pad for the brake pedal. The pedal had bare metal exposed.  b)  Tusk 500PG-16, SN- 205639A. The forklift was defective at the time of the inspection; the forklift LPG locator/indexing pin was in place but not utilized;  the tank was improperly installed. The forklift was missing a pad on the brake pedal.  The pedal had bare metal exposed. c)  Tusk 500CG-14, SN-591158A. The forklift was defective at the time of the inspection; the forklift was missing the LPG locator/indexing pin and the tank was improperly installed. The forklift was missing a pad on the brake pedal. The  brake pedal had bare metal exposed.  One strap was not utilized to secure the propane tank to the forklift.
