29 CFR  1910.178(q)(1): Any power-operated industrial truck not in safe operating condition shall be removed from service. All repairs shall be made by authorized personnel.  On or about August 4, 2022 at jobsite located in 836 N. York Ave. Elmhurst, IL., the employer did not remove from service as required the Pettibone Michigan Tractor Serial#60841CCC Model No. TL6035 that was not in safe operating condition. The seatbelt was defective, employees were exposed to crushing hazards and head injuries in the event of a lift truck tip over.    Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).
