29 CFR 1910.132(d)(1)(i): When the employer had assessed the workplace hazard(s) and determined that hazard(s) were present, the employer did not select and/or use the types of personal protective equipment that would protect the affected employee from the hazard(s): On and about January 23, 2023, the employer did not select and/or require the use of personal protective equipment for the upper extremities (arms) that were not protected by the gloves worn. Employees handle lead containing blasting powder during cup filling and handling resulting in exposure to lead ingestion hazards.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about October 25, 2022, at the capping section of the Stab Det Table, operators were exposed to an ingoing nip point as a conveyed part passed through an opening in the stationary plexiglass guard.
