29 CFR  1926.102(a)(1):The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  a. On or about August 11, 2022, at the above addressed jobsite, an employee using a pneumatic nail gun did not wear safety glasses, thereby exposing the employee to the hazards associated with eye injuries.

On or about August 11, 2022, at the above addressed jobsite, an employee performing residential roofing operations, nine (9') feet five (5") inches above the ground, was not protected from falls, thereby exposing the employee to fall hazards.

On or about August 11, 2022, at the above addressed jobsite, the portable ladder used to access the roof did not extend at least three (3') feet above the upper landing surface, thereby exposing an employee to the hazards associated with falls.
