29 CFR 1910.22(a)(3): The employer did not ensure that walking-working surfaces are maintained free of hazards such as sharp or protruding objects, loose boards, corrosion, leaks, spills, snow, and ice: Midwest Tire Disposal, main work area, stairway to top of shredder: On or about 12 August 2022 and 4 November 2022, the six stairs from the floor to the top of the stairway on the Artech MDE 250-63 Shredding System was covered in an oily residue, tire shards and water. Additionally, wires were also discovered on the concrete floor inside as well as outside near the west side of the building.

There are rotating support rollers on the top and underside of the conveyor belt that have unguarded in-running nip points between the support rollers and belt.
