Lockout or tagout shall be performed only by the authorized employees who are performing the servicing or maintenance. 29 CFR 1910.212(a)(1):Machine guarding was not provided to protect operator(s) and other employees from hazards created by in-going nip points, rotating parts, and/or flying chips and sparks: On or about August 8, 2022, the employer did not ensure employees performed lockout-tagout while performing service to dislodge a stuck pallet on a conveyor assembly between the palletizer and stretch wrapper located within the northeast corner of the packing room. An employee suffered an amputation injury while attempting to move a pallet that had become lodged on the automatic roller conveyor assembly
