29 CFR  1910.28(b)(3)(i):  The employer did not ensure that each employee is protected from falling through any hole (including skylights) that is 4 feet (1.2 m) or more above a lower level by one or more of the following methods listed in 29 CFR 1910.28(b)(3)(i)(A)-(D):  a) Facility, 760 Braddock Avenue, East Pittsburgh, PA:  On or about January 10, 2023, there was an approximately 18' x 6" x 7' feet deep unguarded opening in the metal floor two steps down from the entry control station/panel, underneath the uncoiler.  The slitter line runs 8 hours a day and employees stand at the entry control panel as the coil feeds into the slitter.

29 CFR  1910.141(a)(3)(ii):  Floor(s) of workroom(s) were not maintained, so far as practical, in a dry condition:  a) Facility, 760 Braddock Avenue, East Pittsburgh, PA:  On or about January 10, 2023,  employees are exposed slip hazards because the metal floor at the control panel is slippery due to oil/grease leaking close by from the Yoder slitter machine.  The slitter line runs 8 hours a day and employees stand at the entry control station/panel as the coil feeds into the slitter.
