On or about August 15, 2022, at the job site 34650 Telluride Ln, Zephyrhills, FL 33541, employees were exposed to a fall hazards in that, the ladder's side rails were not extended at least 3 feet above the upper landing surface and the ladders were not secured to the building structure.
