29 CFR 1926.651(c)(2):A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: a) Excavation located at 11500 Davis Drive, Alpharetta, GA 30005: On or about 08/15/2020, the employer exposed employees to cave-in/engulfment hazards while working in an excavation that measured 7-feet deep by 12-feet wide by 100-feet long, in that a same means of egress was not provided inside the excavation while employees were engaged in sewer pipe installation activities.
