29 CFR 1926.100(a):Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets. On or about August 15, 2022, at scaffolding at the front exterior of the building, an employee worked where tools and materials were overhead and was not in use of a protective helmet. The employee was exposed to struck-by hazards.

29 CFR 1926.451(b)(1)(i):Each platform unit (e.g., scaffold plank, fabricated plank, fabricated deck, or fabricated platform) was not installed so that the space between adjacent units and the space between the platform and the uprights were no more than 1 inch (2.5 cm) wide, except where the employer can demonstrate that a wider space is necessary. On or about August 15, 2022, at the front exterior of the building, an employee worked at a scaffold platform that was not fully planked and was exposed to fall hazards.
