29 CFR 1926.651(k)(1): An inspection of the excavations, the adjacent areas, and protective systems was not conducted by the competent person prior to the start of work and as needed throughout the shift: a) Cyberwerx Project USAFA: On or about August 15, 2022, workers were exposed to crushing and engulfment hazards at various times during the work day when the competent person did not perform inspections as needed throughout the day to identify hazards that required workers to be removed from the excavation until corrections were made that allowed for safe entry.

29 CFR 1926.652(b): The slopes and configurations of sloping and benching systems were not in accordance with the requirement options of 29 CFR 1926.652(b)(1), (b)(2), (b)(3), or (b)(4): a) Cyberwerx Project USAFA: On or about August 15, 2022, workers were exposed to crushing and engulfment hazards at various times during the work day when the competent person directed workers to enter an excavation where the combination of sloping, benching, and shielding was configured in an unsafe manner that did not comply with subparagraph (b)(2).
