29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c). (a) GoldStar Excavation and Sewer at 1933 E 16th Street, Greeley, CO 80631: On August 13, 2022, a crew was conducting water line repair at a residential site. Each employee was not protected from cave-in by an adequate protective system, while employees were sitting on the floor of a trench with vertical cut walls, when the east wall caved-in on top of two workers, injuring one and fatally injuring the other.
