29 CFR  1910.147(d)(4)(i):Lockout or tagout devices were not affixed to each energy isolating device by authorized employees:  a) Hobart Meat Saws: On or about August 1, 2022 and times prior, employees preformed maintenance activities such as changing blades on the vertical meat bandsaws.   The employees remove the push start knob from the machines but do not unplug nor lock and tagout the saws.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:  a) Kroger Limited Partnership 1 dba Kroger: On or about August 1, 2022, and times prior, employees perform servicing and maintenance activities such as replacing blades on the vertical meat bandsaws.  The employer did not perform annual periodic inspections of the energy control program to ensure the requirements of the energy control standards are met.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a) Biro Meat Grinder Serial # C9867193: On or about August 15, 2022 and times prior, an employees used a Biro meat grinder to grind meat for customer orders.  The grinder was not equipped with the tray guard to  prevent employee's hands from entering into the grinder bowl and grinder operation.   b) Hobart Vertical Meat Bandsaw Serial # 27-166-975: On or about August 15, 2022, and times prior, employees used a Hobart vertical meat saw to cut steaks and roasts.  The blade guard on the saw was not adjusted to the height/width of the meat exposing employees to the unused portions of the blade.   c) Hobart Vertical Meat Bandsaw Serial # 27-166-970: On or about August 15, 2022, and times prior, employees used a Hobart vertical meat saw to cut steaks and roasts.  The blade guard on the saw was not adjusted to the height/width of the meat exposing employees to the unused portions of the blade.
